Aircrew members represent a unique occupational group exposed to a constellation of known and probable carcinogens, including cosmic ionizing radiation (CIR) at cruising altitude, UV-A radiation penetrating cockpit windows (for pilots), and circadian rhythm disruption from night-shift work and repeated time zone crossing. Whether these exposures translate to measurably elevated cancer mortality has remained a surprisingly difficult question to answer. Prior studies have relied primarily on cancer incidence rather than mortality and have been hampered by confounding from socioeconomic and lifestyle factors. Health screening common in aviation creates a healthy worker effect that suppresses apparent mortality risk, making any observed excess harder to detect and, therefore, more meaningful when found. The study by Patel et al brings important new evidence to bear on this question.
